I think also that his comments about where you live etc. are grounds for immediate termination. At least in my old position as rehab director if anyone made those type of comments taht I remotely felt threatened by then that would be it. I like the idea of having the cops patrol your area a little extra. I doubt he would do anything but worth talking to the police about.
I have unfortunately had a similar experience as I have had to terminate a few people but overall I was clear with them that they are not meeting the job descriptions citing several examples-which you clearly have and usually even the angry ones don't put up a fight. I am sorry you are going through this and that it has caused you so many physical issues. That sucks.
